Missed opportunity following diagnosis of gout

Lipoprotein profiles should be done as part of screening for gout because gout patients often have abnormal profiles, which is a risk factor for cardiovascular disease. A man was diagnosed with gout at age 40. Fifteen years later he had a heart attack. His triglyceride levels were very high, cholesterol levels were mildly elevated, and HDL-cholesterol levels were reduced. Dietary counseling and medication improved these values. However, his gout had not been adequately controlled because he was being under-medicated. He continued to have gout attacks and new joints became gouty although serum urate levels were within normal range. Urate levels fell into the lower half of normal range and gout attacks disappeared once his medication was increased. Gout patients should reduce alcohol and body weight, be medicated to keep uric acid at the lower levels, and be screened at diagnosis for lipid level abnormalities.

Uniform structured formats for scientific communications - how far should we go?
Article Abstract:
Structured formats for presenting research can improve the quality of study reports. Structure forces the researchers to clarify their thinking, include all relevant detail, and exclude irrelevant material. Examples of structure include specifying that study reports contain an introduction, results, methods, and discussion sections. To further the aim of clarity, completeness, and conciseness, the Annals of the Rheumatic Diseases began requiring in 1993 that article summaries include objectives, methods, results, and conclusion. As of 1997, it has adopted the Consolidation of Standards for Reporting Trials format for reporting randomized controlled trials.

The misconduct of redundant publication
Article Abstract:
Publishing duplicate studies in different journals undermines various aspects of research. Several important strategies could help prevent the temptation to publish duplicate articles. These strategies include recognizing the quality rather than the quantity of researchers' work and better defining authorship. Repeated publications strain an already saturated body of research literature and skew future studies that combine data from published studies. A research study published in two separate journals in 1995 has prompted editors to define their standards more specifically.
