Nurse counseling phone lines: triage or trespass?

HMOs and health plans increasingly offer nurse counseling phone lines as an additional service for health plan members. HMOs say the phone lines are a marketing advantage even as they provide useful triage and medical advice for members. Critics claim such phone lines interfere with the physician-patient relationship and that nurses should not be practicing medicine.

Physician Perry Cook decided to work and reside in Landing, Wyoming, following her residency in radiology at Standford University. The opportunity to raise her family on a ranch was a primary reason for moving to Wyoming, but Cook cites the quality of her hospital setting and the patient-focused attitude of other physicians as critical.

The frontier spirit

Managed care and HMOs have barely made any inroads to Wyoming, which remains one of the last states in the US with a preponderance of solo and independent group practitioners. Physicians in Wyoming value their independence, as do the patients they treat. Residents often travel hundreds of miles just to visit a private physician.
