Article Abstract:
The implementation by Royal Hampshire County Hospital, Winchester, UK, of on-line measurement of bed-sore risk through the Hospital Information System, is described. This measurement is included in the screen named washing and dressing, which includes skincare, and the Waterlow risk score is used. The hospital information project team is developing a new assessment pathway for bed-sores, to offer on-line real time information for the hospital in general and also for every patient, replacing surveys on the prevalence of bed-sores.

Intensive pressures
Article Abstract:
Care standards in the intensive care unit (ICU) at St Bartholomew's Hospital, London attempt to prevent the occurrence of pressure sores. The patient's history and complaint, resource deficiencies and staff communication breakdown influence the development of sores. A code of practice attempts to prevent further skin deterioration after admission to ICU. Within six hours of admission nurses aim to assess existing sores, estimate further risk, photograph severe examples and select an aid for pressure relief.

Managing exudate. (Part 2)
Article Abstract:
A description is presented of the techniques which can be used in the assessment and management of exudate from wounds. The use of primary and secondary dressings, and the choice of dressing are discussed.
