Pediatric trauma documentation: adequacy for assessment of child abuse
Article Abstract:
Hospital physicians may not document pediatric trauma sufficiently to differentiate accidental injury from abuse. The medical records of 642 children treated at one hospital for injury gave no indication of how the injury occurred 4% of the time, did not document a complete examination 33% of the time, reported color of the injury, an indication of injury age, only 9% of the time, and failed to provide enough information to determine child abuse or neglect 6% of the time. Three case reports illustrate suspicious instances in which documentation was inadequate and there was no follow up.

Noncompliance with scheduled revisits to a pediatric emergency department
Article Abstract:
Many parents may fail to bring children treated in the emergency room back for scheduled revisits, and doctors may not be able to predict which parents those will be. This could affect the decision to admit a child who requires follow up. Researchers interviewed the parents of 179 children and the doctors in an urban pediatric emergency room. Half the parents failed to return for revisits, including half the 124 patients doctors were sure would return. Parents judged unable to recognize worsening illness were twice as likely to fail to keep follow-up appointments.
