Physicians, hospitals to share Medicare pay; bundling demo seeks savings, better coordination of care

Physicians and hospitals plan to split Medicare admission payments under a new bundled payment system that the US Health Care Financing Administration will introduce. The lump sum payment would pay for all inpatient services and the division of each payment will be decided by the joint physician-hospital groups. There are eight sites in New Jersey, Pennsylvania and New York that have been selected for the demonstration.

User fees established in the federal government's fiscal 1999 budget are expected to fund fraud and other regulatory procedures of the Health Care Financing Administration. Medicare physician enrollment fees are expected to generate $20 million per year. Fees for claims submitted on paper would raise about $600 million and charges for Medicare certification would raise $330 million, both over a five-year period.
