Recording risk factors
Article Abstract:
Patients at risk to pressure sores should be identified by nurses and steps should be taken to prevent such wounds from occurring, as stated by the UKCC Code of Professional Conduct. There are various risk-assessment scoring systems available or nurses can simply use their own judgement. It is suggested that a documented method is more favourable, in order to allow the nurse to demonstrate the patient's risk to higher management.

Pressure sore assessments. (pressure sore classification and risk assessment)
Article Abstract:
Classification systems have been developed to define pressure sores, prevent their occurence, and identify patients prone to them. The Stirling Pressure Sore Severity Scale (SPSSS) provides a classification system for nurses which is compatible with the recently published staging system by the AHCPR in the US. Staff must be able to determine the implications of individual scores when undertaking assessments.

The desire to obtain qualifications in nursing is overshadowing the need to gain practical experience. Many newly trained nurses appear to be well trained and experienced on paper but find the challenge of working on a ward with real patients much harder than they initially thought. Nurses do not need a degree in nursing to practice, just good common sense, reliable training methods and support.
