Risk factors for HIV infection among young adult men in northern Thailand
Article Abstract:
The incidence of HIV infection is high among young adult men in northern Thailand. Efforts should be made to increase both education about regular condom use and prevention and treatment of sexually transmitted diseases (STDs). Among 2,417 northern Thai men between 19 and 23 years old in the Royal Thai Army or Royal Thai Air Force, 12% were HIV-positive. HIV infection was associated strongly with heterosexual activity. Most of the men had never had sex with another man or used intravenous drugs. Sexual contact with female prostitutes was reported by 97% of HIV-positive men, compared with 79% of the men who were HIV-negative. Seventy-seven percent of the HIV-positive men had had a STD, compared with 38% of the HIV-negative men. Sixty-two percent of the men had used a condom the last time they had sex with a female prostitute.

Dynamics of risk behavior for HIV infection among young Thai men

Many young men in Thailand have not changed their high-risk behaviors substantially. Researchers gave blood tests and surveys of sexual behavior to 897 young Thai soldiers every six months for an average of two years. Prior to their army service, 42% of the men had never visited a prostitute, but two-thirds of these men did so during their active duty. In the year before conscription, 17% visited a prostitute at least once a month and one-third went less frequently than once a month but more often than once a year. Fewer than 20% of the men never visited a prostitute during their service. Many of the men increased their frequency of sex with prostitutes. Many of the HIV-positive men continued to visit prostitutes. In Thailand, one of the most important risk factors for HIV infection is having regular sex with a prostitute.

Risk factors for HIV-1 seroconversion among young men in Northern Thailand

Sex with female prostitutes appears to be one of the major risk factors for HIV infection in young men in Northern Thailand. Researchers followed 1,932 men from six provinces in Northern Thailand who were drafted into the Royal Thai Army and Air Force in April 1991. At that time, 289 (12%) were HIV-positive. The men had at least one semi-annual follow-up visit. Eighty-five HIV-negative men became HIV-positive during the study. Risk factors for seroconversion included substance abuse, sex with prostitutes, sex with men and a history of a sexually transmitted disease. Seventy-three percent of the men reported visiting a prostitute at least once during their service. Those who visited prostitutes frequently were more likely to become infected.
