Expanding horizons
Article Abstract:
The extended role of nurse practitioner (NPs) has prompted mixed reactions. The NP provides a more holistic approach to nursing care. However, some feel it challenges the power of the medical profession by devolving tasks traditionally carried out by doctors. The role has developed slowly due to bureaucratic uncertainties. However, it could aid the professionalisation of nursing. The autonomous NP challenges nurses' traditional submissive role and may increase self-esteem. Government support for the initiative may encourage NPs to be more assertive in the health care system.

Taking the measure of patient satisfaction
Article Abstract:
Nurses at West Suffolk Hospital, Bury St Edmunds, UK, developed a programme to assess nursing care quality. The UK government has placed an emphasis on the quality of care in the National Health Service. Patient satisfaction is a key element in assessing quality, but measurement of satisfaction is difficult due to the subjective element. The West Suffolk programme addressed this problem by first eliciting patients' expectations of nursing care. They then compiled an audit questionnaire which showed standards of care to be expected before seeking patients' experience of care.

Where naming cuts confusion
Article Abstract:
The use of named nurses in accident and emergency (A&E) units is difficult due to rapid and unpredictable patient admissions. The seriousness of ailments varies and little time is available to form relationships. However, the assignment of named nurses improves the coordination of patient care. A personalised service is less fragmented and increases patient satisfaction. Greater responsibility for nurses leads to improved morale. Case records may also be improved.
