Teaching residents about patient and practice termination in community-based continuity settings
Article Abstract:
Residents and their on-going patients in community practices may benefit from a structured set of practice termination strategies at the end of their residency. The pediatric residency program at the University of Massachusetts Medical Center in Wooster assigns pediatric residents to community clinics during their three-year residency. At the end of the residency, residents must separate from patients, office staff, their preceptor, and make the transition to a new stage in their careers, all of which raise logistic and emotional concerns. Program directors have facilitated this process by holding a senior seminar to discuss termination, allowing extra time for termination visits with patients, and holding a luncheon with office staff and a dinner with preceptors. All of these interactions provide opportunities for discussing and addressing the issues of detaching from patients, instructors, and staff.

A methodology to maximize resident input in improving a pediatric rotation
Article Abstract:
Faculty from the pediatric residency program at The Children's Hospital of Philadelphia, PA, developed a survey and discussion technique to solicit the perspective of residents on the educational experience. The survey was returned by 51 of 80 pediatric residents, who then participated in group sessions to prioritize problems and generate solutions. Residents were concerned about pressures to treat patients rapidly, strained relations with nursing staff, paperwork, feedback, and other problems. The method incorporates elements of focus group, Delphi, and Nominal Group techniques.
