The 1987 US hospital AIDS survey
Article Abstract:
In 1987, the National Public Health and Hospital Institute questioned 632 acute-care hospitals around the nation concerning inpatient and outpatient care for patients with AIDS. Of those, 276 reported treating patients with AIDS, who were hospitalized an average of 16.8 days. The average cost per inpatient per day was $681, average revenues were $545, and the average cost per patient per year was $17,910. The estimated cost of care for AIDS inpatients during 1987 was $486 million, with Medicaid was the primary payer. Hospital utilization, payers, and financing varied according to region of the country and type of hospital ownership. Some hospitals had much greater losses than others due to the payer source and patients' lack of insurance. Many hospitals that serve large numbers of poor people with AIDS had moderate to severe financial shortfalls. Increasing concentrations of people with AIDS in relatively few hospitals in the inner city may make it more difficult to obtain the broad political base needed to obtain adequate support for treatment.

Comparisons of hospital care for patients with AIDS and other HIV-related conditions
Article Abstract:
Individuals who have an HIV-related disease but have not been diagnosed with AIDS may represent as much as one-third of the admissions for HIV infection. A survey of 325 hospitals that treated at least one patient with an HIV illness other than AIDS (OHIV) found that they represented 35% of all admissions and 29% of all costs. For every 100 AIDS patients admitted, 53 OHIV patients were admitted. Compared to AIDS patients, a greater number of OHIV patients were likely to lack insurance. Although AIDS treatments cost 14% more per day, a greater number of OHIV patients would require charity care. Consequently, many private hospitals may be unwilling to treat these patients. The Centers for Disease Control recently proposed changing the definition of AIDS, which would result in many more OHIV patients being diagnosed with AIDS.

Averting traumatic injury to trauma care in Los Angeles: a near miss with lessons for all
Article Abstract:
Local communities must do whatever they can to ensure the survival of their trauma centers. Many Americans believe trauma centers only serve the poor and uninsured but the closure of many centers will affect rich and poor alike. Many trauma centers are located in public hospitals that have been affected by managed care. Managed care can draw paying customers away from public hospitals, leaving them with a greater burden of uncompensated care. Parkland Memorial Hospital in Dallas is an example of a public trauma center that has renewed its efforts to provide adequate trauma care.
