The effect of link nurses on hospital readmission rates
Article Abstract:
A study examines whether visits to a patient early post-discharge by a member of the ward team (locality link nurse (LLN)) reduces psychiatric hospital readmission rates for older people and improves the psychological state of the patient. It is concluded that an LLN visitation two weeks post-discharge is not an effective solution as it was found the inability of LLN intervention to reduce hospitalization is congruent with the observation of no significant change in cognition, depression or individual needs for the follow-up group.
