Thoughts after birth
Article Abstract:
Birth Afterthoughts, a service set up in Oxford, England, in 1994 by midwifery lecturer practitioner Julie Smith, aims to help women with inquiries about birth trauma, sometimes many years after the incident took place. In the first year, 46 women approached the service. They wanted to discuss many different issues, including failures in communication, problems with meeting their expectations and the need for information. The information obtained by the service is used to improve care, and is conveyed to a range of professionals.

Care management in the community: a case study
Article Abstract:
The UK NHS and Community Care Act of 1990, which should have created a reformed environment for better patient care, is not working in practice. The collaboration which is meant to occur between the NHS and the local authorities has still resulted in poor communication, cooperation and effectiveness at providing the support and services that patients need. Care management and consumerism should be on the education agenda for staff on the ground level for the community care reforms to work.

Caught unawares
Article Abstract:
A British Medical Association survey has suggested that General Practitioners (GPs) are generally ill-prepared for the implementation of community care on Apr 1, 1993. Nurses are worried that GP purchased nursing services could be inadequate. Many fear a possible increased workload of 30% may result in nursing waiting lists. Extra resources to meet increased needs are unlikely.
