Transatlantic challenges
Article Abstract:
A study tour of North American and Canadian hospitals has established practice relevant to cardiothoracic nursing in the Royal Brompton National Heart and Lung Hospital, London, England. Case management and primary nursing provided a model for a thoracic nurse specialist. A telephone help-line based on American practice has been introduced. American emphasis on research has prompted teaching sessions. American practice has also influenced developments in pain control, community liaison, information and support.

Making sense of mini-tracheostomy
Article Abstract:
Mini-tracheostomy is a surgical procedure involving the insertion of a small-bore tube into the trachea. It is a common response to inefficient expectoration following surgery. Only local anaesthetic is necessary. Possible complications include respiratory distress following insertion and infection. Advantages over standard tracheostomy include less patient distress and normal glottis function.

Pre-admission clinics for thoracic surgery
Article Abstract:
The introduction of a pre-admission service for thoracic surgery at Royal Brompton Hospital has had positive effects. A cardiothoracic coordinator helps make the hospital stay more positive for the patient and relatives, by informing them about surgery. Discharge delays can be avoided by assessment of the home and discharge planning in clinic.
