Dr. John Gocke and nurse practitioner Kathy Smeltzer practice together at the Carle Clinic Assn in Urbana, IL, a group of about 220 physicians. Nurses have complained in the past that their treatment by physicians caused them distress. The situation has changed, according to Gocke, who values the contribution an intelligent nurse makes to a practice. Smeltzer handles routine examinations and treatments, and Gocke can focus on critical care. Carle Clinic has acquired additional doctor-nurse practitioner teams.

Physicians should identify patients who smoke and offer help in breaking the habit, according to guidelines of the Agency for Health Care Policy & Research. More than 70% of smokers claim they want to quit, but the majority report that their doctors have not offered advice. Even a three-minute talk can help patients choose to participate in treatment for smoking cessation. Managed-care plans should cooperate in paying for cessation treatment, because long-term health costs will be lowered.

Dietary habits have been linked to five of the ten leading causes of death, and good diets can play significant roles in the management of chronic diseases. However, many physicians are not trained to identify nutritional or dietary deficiencies, and even fewer spend time with patients for nutritional counseling. Nutritional counseling can contribute cost savings for managed care health plans.
