Article Abstract:
The case of a transsexual at aspecial hospital in Merseyside who was given only limited opportunities to dress as a woman, isdescribed. Despite repeated requests for treatment the patient remained an unfulfilled transsexual, and made a series of formal complaints. The patient's difficulties were compounded by beingplaced in an all-male patient population and there was considerable opposition from hospital staffto any attempts to meet the needs of the patient. The patient attended a rehabilitation centre andwas later discharged to a residential care home for older people, where she is functioning well inthe community.

Outreach care: A sensitive approach
Article Abstract:
Assertive outreach teams envisaged by health secretary Frank Dobson would be based on the assertive community treatment (ACT) model. The aim is to help those with serious and persistent mental illnesses to live stable and meaningful lives. Behind the objective is the implicit requirement to protect both patients and the public from the risk of violence, but there are no claims that the model reduces such risks. An alternative model has been developed known as active client engagement (ACE), which aims to encourage collaborative relationships with a focus on sound and trusting therapeutic relationships.
