Uterine activity after preterm premature rupture of the membranes

Premature rupture of fetal membranes before full term is not common, but can cause serious complications and even death. This complication of pregnancy is associated with respiratory distress syndrome, which affects 10 to 40 percent of newborns, and results in death in 20 to 50 percent of cases. The cause of preterm premature rupture of membranes is not known, but may be related to several factors, such as distention of the uterus, impaired function of the cervix (the opening of the uterus), abnormal positioning of the fetus in the uterus, infection of the genital tract, or defects in the fetal membranes. The relationship between increased activity of the uterus and preterm premature rupture of membranes was assessed in 101 women. The women had preterm premature rupture of membranes between 26 and 34 weeks of pregnancy. Rupture occurred at an average of 31.4 weeks of pregnancy, and delivery occurred at an average of 33.7 weeks of pregnancy. The frequency of uterine contractions increased significantly 24 hours before the start of labor. The occurrence of four or more uterine contractions was 72 percent sensitive and 90 percent specific in predicting the start of labor within 24 hours. Women with ruptured membranes had a similar frequency of uterine contractions as women without ruptured membranes who also entered into premature labor. Birth weight references for triplets
Article Abstract:
The purpose of this study was to formulate growth references that reflect triplet fetal and neonatal populations at each gestational age by combining serial ultrasonographic estimates of fetal weight and measured birth weights. These data confirm in triplets what we have previously demonstrated in twins, that, in utero, well-grown children have similar growth potentials, regardless of plurality.

Clinical and sonographic estimation of fetal weight performed during labor by residents
Article Abstract:
A study was undertaken to assess the accuracy of both clinical and sonographic estimations of the fetal weight (EFW) performed during the active phase of labor by residents. Both clinical and sonographic EFW performed during the active phase of labor by residents correlate with the actual birth weight but have poor sensitivity in detecting macrosomic fetuses.
