UK Audit Commission for Local Authorities in England and Wales controller, Andrew Foster is enthusiastic about the implementation of community care on Apr 1, 1993. He considers it the only realistic response to increased demand for care by an ageing population. Emphasis will be on needs assessment. However, initial problems are inevitable. Bed blocking may occur without adequate discharge arrangements. Contingency plans should be established for medical and economic crises.

Implementation of community care on Apr 1, 1993 will necessitate close cooperation between social service departments and health authorities. Many authorities already operate collaborative services. Mutually-agreed discharge plans are in place and purchaser/ provider working relationships have been established. Effective consultation and service monitoring should prevent bed blocking. However, many fear inadequate funding may ultimately threaten commnuity care.

Article Abstract:
Community care services for those with disabilities and the elderly are being cut or reduced UK-wide as a result of local authority over-spending or lack of funds. The Association of County Councils estimated that at the end of 1994, 24 local authorities had financial difficulties. Bed-blocking has been reported as a growing problem, and voluntary organisations, such as Age Concern, are increasingly burdened as a result of the cutbacks.
