Where there's a will

The increased use of living wills, or advance directives, by the terminally ill in the United Kingdom has important implications for nursing staff. Once a patient has made a decision to create a living will it is vital that it is witnessed by a doctor. Nurses must be assured that they are not involved with any form of euthanasia but they are permitted to withhold certain treatments if it is a written directive from the patient. A medical alert card could warn nurses who do not have access to the full medical records of a patient's wish to stop treatment.

Primary care groups (PCGs) were set up in April 99 and there are 400 of them across the United Kingdom. Nurse representatives have had to learn about health improvement programmes, joint investment plans and performance assessment frameworks. It is important that nurses acquaint themselves with the representatives on the PCGs and tell them what they think patients' needs are. PCGs are also an important means of negotiation so that an effective service can be offered to patients.
