When human error can kill
Article Abstract:
The healthcare industry is a complex system, like the aviation industry, in which mistakes are inevitable, and the consequences of those mistakes are potentially disastrous, and there is evidence that modern health care is a major cause of death and disability. Aviation experts suggest that everybody in the health care system must make safety a priority, and must report mistakes without retribution.

Harmful lessons
Article Abstract:
The letter from the Ministries of Youth Affairs, Education and Health, New Zealand, warned that some approaches to youth suicide-prevention, albeit well mentioned, are harmful and can increase suicide risk. The letter followed concerns that some New Zealand students' deaths and suicidal behaviour appear related to the proliferation of suicide-prevention programs in schools.
