Article Abstract:
Issues are discussed regarding the rise in the 1990s of Medicaid managed care and the decrease in enthusiasm for this approach. The technical and political issues involved in extending managed care to those with special needs are listed and the decreased momentum in Medicaid managed care due to the exit of commercial firms from this market discussed.

Article Abstract:
Consumer pressure and well-thought out regulatory authority as engines driving needed reform in managed care are discussed. The maturation of the industry as these two enines lobby for needed reform will lead to further changes.
