Case management does not appear to substantially reduce the cost of health care or the use of medical services by elderly patients, according to a study of 6,409 aged patients. Case management involves using social workers or nurses to coordinate a patient's medical, social, and family needs.

Using self-reported data to predict expenditures for the health care of older people
Article Abstract:
A study of the efficiency of using medical cost information self-reported by elderly patients to predict Medicare costs is examined. The study found that self-reported costs produced more accurate predictions of Medicare costs than the current principal inpatient diagnostic cost group method of prediction.

Improving the quality of transitional care for persons with complex care needs
