A macro-ergonomic work system analysis of the diagnostic testing process in an outpatient health care facility for process improvement and patient safety
Article Abstract:
A study was conducted to examine the diagnostic testing process and a macro-ergonomic work system analysis was used to uncover system flaws that contributed to delayed physician and patient notification of results. The results indicated a number of variances that contributed to delays, the majority of which occurred across the boundaries of different systems and were related to poor or absent feedback structures, and recommendations for improvements are discussed.

From cognition to the system: Developing a multilevel taxonomy of patient safety in general practice
Article Abstract:
The process of developing a taxonomy of patient safety in general practice is described and the methods employed included fieldwork, task analysis and confidential reporting of patient-safety events in five West Midland practices. The resulting taxonomy is based on a theoretical model of human cognition, includes multiple levels of classification to reflect the chain of causation and considers affective and physiological influences on performance.
