Maine helicopter pilot Sean Rafter, owner of organized air ambulance firm Airmed Skycare Inc., lost his Maine air ambulance license after he crashed into the Casco Bay in Portland, OR, while on duty. He was found to have used poor judgment which led to the deaths of two crew members and one patient.
