Los Angeles; temblors in Lotus Land; lessons for other markets abound in this laboratory of system change
Article Abstract:
Coming changes in the delivery of health care in the Los Angeles metropolitan area foreshadow events in the rest of the United States. Managed care systems have caused many community hospitals to close, and overcapacity threatens more closures. Emergency services will suffer if public hospitals close. Hospital mergers are forecast, and large academic medical centers such as UCLA are building networks of satellite clinics. Kaiser Foundation Health Plan Inc. and other health maintenance organizations are planning money-saving strategies.

Employer costs and consumer attitudes toward health maintenance organizations (HMOs) are causing employers to seek alternative healthcare plans. Many employers are beginning to contract physicians on a negotiated fee-for-service basis. This type of plan allows patients to chose their physicians directly while decreasing payer costs. Other employers are contracting with companies which administer benefits through HMOs but allow employees to chose their independent health care deliverer.

Hospital Research and Educational Trust Chmn C. William Pollard believes that the growing assertiveness of health care insurers is becoming a major issue in the industry. Some fundamental changes are being initiated by payers' insistence on higher quality care at lower costs. Payers are also changing their definitions of community, which in turn, is changing the way that providers operate.
