Article Abstract:

Several states are considering pooling their Medicare and Medicaid funds to provide a comprehensive range of health care services for eligible senior citizens. Minnesota has received federal approval for a program that may start in late 1996. State officials want to stretch health-care budgets as far as possible. More than 5 million seniors in the US are eligible for both Medicaid and Medicare. Health care providers worry that quality and reimbursements will decline under a combined system.

Playing second fiddle: fitting into the future is the biggest challenge facing today's urban hospitals

Article Abstract:

Hospital mergers and consolidations are causing difficulties in urban hospital markets because urban hospitals are not participating in the mergers. As a result, there could be a lack of adequate health care for low income and minority communities. Urban hospitals are under pressure to decrease public expenditures, have had to decrease public services and must provide services for a higher amount of uninsured patients.

Unintended targets

Article Abstract:

Federal budget cutters will ultimately succeed at reducing government healthcare spending, but spending cuts will have unintended consequences. Cost-shifting will intensify, and hospitals with many Medicaid and Medicare patients will become unattractive to private payers, because they will have to charge private payers higher prices. Several other projected consequences are discussed.
