PPS rule proposes changes in reclassification, outlier policies
Article Abstract:
Hospitals may expect substantial changes in Medicare reimbursement due to proposed Health Care Financing Administration rules which take effect October 1, 1993. Hospitals would be reclassified to one geographic area for both wage indexing and standard payment amounts. Outlier policies would be revised to pay less for longer and more expensive hospital stays.In a survey by the American Hospital Assn, 35% opposed any change and supported the proposed regulation, in part because the wage index is to be totally revised in 1994.

The American Hospital Association (AHA) is trying to overturn legislative provisions that are producing significant shifts of money from urban to rural hospitals because of Medicare geographic reclassification rules. The Medicare Geographic Classification Review Board, created by Congress, is responsible for reducing payment inequities resulting from hospitals being identified with job markets that do not reflect their true costs or wages.

HCFA moves slowly on federal outpatient payment reform
Article Abstract:
The Health Care Financing Administration (HCFA) failed to implement its Medicare hospital outpatient payment reform proposals by the Sep 1991 deadline. HCFA research continues on ambulatory patient groups, the outpatient equivalent of inpatient diagnosis-related groups. In addition, the HCFA has yet to finalize a 1988 hospital outpatient bundling regulation.
