Not-for-profit hospitals have begun seeking joint venture deals with for-profit companies, but some community leaders feel that the arrangement benefits the stockholders more than the community. Such arrangements usually result in a fifty-fifty sharing of assets and responsibilities, but the realistic scenario points toward the board members controlling the majority of the funds. Most ventures will function according to the rules of for-profit companies, and charitable concerns will likely be discarded.

Negotiations between not-for-profit and for-profit healthcare organizations concerning acquisitions must take into account the fiduciary responsibilities involved, which include financial, constituency, and legal considerations. Board members are required to keep the interests of the organization above their own private interests or concerns. Board members should also be aware of the constituencies being served by the hospital, and must seek to deal fairly with all sides.

Non-profit hospital ventures, which seek to sell their organizations to for-profit hospitals, are being scrutinized for the manner in which they deal with their responsibility to constituents. The sale of Rose Health Care System in Denver, CO, was applauded for the directors' decision to convince the buyer to continue Rose's charitable work. A similar sale of HealthONE, also in Denver, was criticized by community groups for failing to provide for charitable efforts.
