A 78-year-old woman with increasing dysphagia and odynophagia
Article Abstract:
A 78-year-old woman with difficulty and pain in swallowing was found to have Crohn's disease of the esophagus. Crohn's disease involves the inflammation thickening of the wall of the digestive tract, often resulting in blockage. The patient had been previously diagnosed with a hiatal hernia of the esophagus, reflux of stomach enzymes, and irritable bowel syndrome. A CT scan, x-rays, and examination of her esophagus with an endoscope indicated an obstruction of the esophagus, thickening of the esophageal wall, and swollen lymph nodes in the area. To follow up a biopsy that suggested cancer, a larger endoscope was put down her esophagus and perforated the wall of the tube. A section of the esophagus was surgically removed and found not to contain cancerous cells but to be affected by Crohn's disease. The patient returned home, no longer had difficulty swallowing, and refused to take corticosteroids.

A 54-year-old woman with a progressive gait disturbance and painful leg paresthesias
Article Abstract:
A 54-year-old woman was admitted to a hospital with pain and abnormal sensation in her legs and difficulty walking. A chest X-ray revealed a mass in her lung and an MRI of her brain revealed abnormalities that could have accounted for her neurologic symptoms. Her physicians suspected sarcoidosis as the most probable diagnosis. Sarcoidosis usually affects the lungs and can spread to the brain. A biopsy of a lymph node confirmed the diagnosis.
