Assessment and prediction of long-term cure in patients with the Zollinger-Ellison syndrome: the best approach
Article Abstract:
The secretin provocative test and fasting blood gastrin determination may the most effective methods for early diagnosis of recurrent Zollinger-Ellison syndrome after surgical removal of gastrinoma. Zollinger-Ellison syndrome is a disorder characterized by peptic ulcers, increased acidity of the stomach and gastrinoma. Gastrinoma are non-beta islet cells tumors of the pancreas that secret gastrin. Among 81 Zollinger-Ellison syndrome patients who had exploratory surgery, 96% had gastrinoma. Fifty-two percent of the patients were free of disease immediately following surgery, 42% after one year and 35% after five years. A recurrence was first detected using secretin provocative testing in 45% of the patients with recurrent disease. Blood gastrin determination was the first test to detect a recurrence in 36% of the patients with recurrent disease. Imaging studies and calcium provocative testing did not initially detect a recurrence in any of the patients.

Somatostatin receptor scintigraphy: its sensitivity compared with that of other imaging methods in detecting primary and metastatic gastrinomas: a prospective study

Somatostatin receptor scintigraphy appears to be the most accurate means of locating gastrinomas in patients with Zollinger-Ellison syndrome. These patients tend to develop gastrinomas, a type of tumor. Eighty patients with Zollinger-Ellison syndrome underwent ultrasonography, computed tomography, magnetic resonance imaging, selective angiography, bone scanning, and somatostatin receptor scintigraphy. Somatostatin scintigraphy identified 58% of primary tumors and 70% of metastatic tumors whereas the next highest percentages were 31% and 45%, respectively, for other imaging methods. Nonetheless, the technique missed 20% of gastrinomas found during surgery.

Surgery to cure the Zollinger-Ellison syndrome
Article Abstract:
Many patients with Zollinger-Ellison syndrome can benefit from surgery. Patients with Zollinger-Ellison syndrome have tumors in their gastrointestinal tract that can cause peptic ulcers. In a study of 151 patients with Zollinger-Ellison syndrome who had surgery, 40% of the patients with a single tumor were still free of disease five years later and 34% were free of disease 10 years later. Patients with multiple endocrine neoplasia type 1 or metastatic cancer did not fare as well.
