Bilateral ovarian neoplasms and the risk of malignancy
Article Abstract:
It has been widely believed that ovarian tumors are more likely to be malignant if found in both ovaries than if occurring in only one. The evidence for this impression has been largely anecdotal. To rectify this situation, medical records from a ten-year period were reviewed and 861 patients with ovarian tumors were identified. Of these, 189 tumors were bilateral. The total numbers of malignant tumors were 104 unilateral and 76 bilateral. Thus, the chance of a tumor being malignant is much greater if it is occurs in both ovaries. The relative risk factor varies with age, however, and it is a somewhat confounding factor. The relative risk in premenopausal women was 2.8 as opposed to 1.6 in post menopausal women. Since the overall rate of malignant tumors, unilateral or bilateral, is higher in older women, the relative risk factor obscures the importance of bilaterality as a factor in older women. The absolute increase in risk associated with bilaterality is 23.6 per 100 women in postmenopausal women, as opposed to 17.7 per hundred in premenopausal women. The bilaterality is thus a clinically more significant observation in older women than in younger. Knowledge of the probabilities involved will permit more effective counselling of patients in preparation for surgery. (Consumer Summary produced by Reliance Medical Information, Inc.)

Primary stress urinary incontinence and pelvic relaxation: Prospective randomized comparison of three different operations
Article Abstract:
Stress urinary incontinence is the involuntary passing of urine due to stress. Failure of bladder support and pelvic relaxation can proceed to cause the bladder and uterus to fall from their original positions. Surgery can be performed to lift and stabilize the affected organs. Three different operations were compared in 289 women. The Burch retropubic urethopexy procedure had better cure rates after one year than either Pereyra or anterior colporrhaphy methods.

Evaluation of atypical glandular cells of undetermined significance: is age important?
Article Abstract:
Older women who have a Pap smear identified as atypical glandular cells of undetermined significance have a higher risk of uterine cancer than younger women. Any woman over the age of 45 who has this type of Pap smear should have an endometrial biopsy.
