Blood transfusion

The US blood supply is now so safe that the decision to transfuse blood will be determined by cost considerations and whether the patient needs a blood transfusion. The number of units of blood transfused in the US rose from 10 million in 1980 to 12.2 million in 1986, and then dropped to 11.4 million in 1997. Part of the drop resulted from the fear of contagious diseases transmitted through blood products, including HIV, hepatitis B and hepatitis C. Blood transfusion can cause other adverse reactions. Some patients may be getting blood unnecessarily whereas others are not getting the blood they need.

Erythropoietin therapy
Article Abstract:
Erythropoietin could be given to some surgical patients to eliminate the need for blood transfusions. Erythropoietin is a naturally produced hormone that stimulates blood cell production. Patients with pre-operative anemia or those who will lose 1 to 3 liters of blood during surgery may benefit from pre-operative erythropoietin. Pre-operative erythropoietin could also be combined with autologous blood donations, in which patients donate blood that will be given back to them after surgery. Erythropoietin can be effective even in patients who do not donate autologous blood.

Anemia of chronic disease, the anemia that is the second most prevalent after anemia caused by iron deficiency, occurs in patients with acute or chronic immune activation. The most frequent conditions associated with anemia of chronic disease are listed.
