Case management: considerations for coordinating quality services in rural communities
Article Abstract:
Case managers in rural communities are faced with different problems than those in urban areas. Scattered population, reduced number of formal health care services, and increased number of governmental regions within the service area are a few of the challenges rural case managers face. The most effective rural case managers tend to be people who have resided in the community for most of their life. Their understanding of the population's culture and local politics is an asset to their work.

Empowering initiatives to improve a community's health status
Article Abstract:
The community health assessment process can help answer various pertinent questions for a community's medical providers so that they can provide both better-quality medical care and better preventive health education. Personal interviews with community members and small focus groups are two ways of obtaining more information about a community. Health is promoted through collaborative working relationships between medical providers and a community.

An integrated delivery system will facilitate patient progression through a medical care system. Mount Sinai Hospital in New York City is streamlining their health care system by focussing on the primary care nurse who gives direct care in cooperation with the interdisciplinary health care team.
