Circadian rhythm of serum cytidine deaminase in patients with rheumatoid arthritis during rest and exercise

Rheumatoid arthritis (RA), an inflammatory joint disease, is characterized by swelling, stiffness, hypertrophy (enlargement) of the cartilage, and pain. Clinical measures of disease activity in RA, such as grip strength, pain, and stiffness worsen in the early hours of the morning. Similarly, some indicators of inflammation, such as the total amount of blood proteins, and levels of transferrin, which carries iron and alpha-1-antitrypsin which blocks enzymes that degrade proteins, show daily changes in levels, whereas other inflammatory markers, such as C reactive protein, do not exhibit diurnal changes. Cytidine deaminase is an enzyme released from dead or damaged polymorphonuclear leukocytes, a type of inflammatory cell, in inflamed rheumatoid joints. The enzyme diffuses from the synovial (joint) fluid to the blood. The blood levels of cytidine deaminase reflect the extent of joint inflammation and the patient's condition. The circadian (24-hour) changes in cytidine deaminase and C reactive protein levels were measured in 11 patients with RA. There were changes in 24-hour cytidine deaminase levels after activity but not at rest. The peak level occurred at 1,208 hours, after a period of morning physiotherapy. This suggested that exercise causes the circadian variation, probably by increasing the removal of cytidine deaminase from inflamed joints by the lymphatic system. C reactive protein showed no changes in levels over a 24-hour period, consistent with previous findings. (Consumer Summary produced by Reliance Medical Information, Inc.)

A Dutch quality of life questionnaire, or the IRGL, may be an effective method for evaluating patients with rheumatoid arthritis (RA). The IRGL measures the influence of rheumatoid arthritis on healthcare and lifestyle. RA is an inflammatory disorder that affects the joints. Researchers evaluated 282 RA patients using the IRGL and different clinical and laboratory tests. An association was found between the clinical and laboratory measurements and the physical dimension and disease impact scales of the IRGL. The psychological scale of the IRGL correlated less with the results of the clinical and laboratory tests. No significant correlation was found between the clinical and laboratory measurements and the social scales of the IRGL. The IRGL provides a more complete assessment of the health status of RA patients because it also evaluates psychological and social factors.

Effect of circulating immune complexes on the binding of rheumatoid factor to histones

Rheumatoid factor does not appear to bind directly to histones but rather to immune complexes that are bound to histones. Histones are proteins associated with DNA in the cell nucleus.
