Dying at home: Supporting patient choices
Article Abstract:
Patient-focused care should mean that patients should have a say about where they die, but research suggests that patients are not always able to die in the place of their choice. An out of hours palliative care district nursing service was set up to give support to patients who wanted to die at home. The pilot service was evaluated using Priority Health Care Wearside NHS Trust's audit framework. 37 of the 80 patients referred to the service during the pilot period died, 31 of those at home.

Success of primary nursing as an organisational tool depends on those using the system. Efficiently implemented primary nursing increases nurse and patient autonomy with a consequent improvement in health care. However, misunderstanding may occur over leadership, workload, nurse/patient relationships and patient autonomy. Uncritical adoption of primary nursing techniques will not automatically solve all problems.
