Improving palliative care
Article Abstract:
Much progress is needed to make sure that hospitals provide adequate palliative care for the terminally ill. Most deaths occur in hospitals, and the failure of hospitals to provide adequate end-of-life care has probably inspired the physician-assisted suicide movement. Many physicians believe that they have failed when their patient dies. These attitudes need to be changed and medical students, residents and physicians need to be properly trained in the provision of end-of-life care. The health care system could also be reorganized to integrate hospice services more completely.

A Consensus-Based Approach To Providing Palliative Care to Patients Who Lack Decision-Making Capacity
Article Abstract:
A method is described for doctors and family members to work together on behalf of patients who can no longer make decisions for themselves. It uses a case study of a patient with Alzheimer's disease to illustrate various techniques. Readers will learn a consensus-based decision-making strategy that keeps the patient's wishes and values, but also expects guidance from the physician and input from family members and other people who care for and have knowledge about the patient.

Discussing Palliative Care with Patients
Article Abstract:
Two hypothetical cases are described to help doctors discuss palliative care with the relatives of dying patients. Palliative care does not cure the patient but focuses on relieving suffering, providing psychosocial support, and preparing the family and the patient for death. Doctors should ask patients open-ended questions and respond to the patient's concerns before discussing specific clinical decisions. Responses to difficult questions are provided.
