Managing chronically ill older people in the midst of the health care revolution
Article Abstract:
Continuum-based care and nurse case management are critical to the provision of quality health care to the chronically ill older population in a changing health care environment. Case management is a strategy that coordinates health care across a continuum. An emerging model of nursing practice gives nurses increased authority for making decisions. The relationship between the nurse and the chronically ill person involves bonding, working and changing.

Nurses' and physicians' valuation of and willingness to promote teamwork
Article Abstract:
There has been a worldwide movement to redesign health care delivery system and integrate health professionals into coherent teams. The nurses considered teamwork more important and appreciated their hospital's promotion of teamwork more than the physicians. Both groups seemed equally willing to promote teamwork.
