UK district nurses may be under greater pressure due to the shortening of inpatient treatments. This situation, and the focus on contracts between health authorities and trusts, has created a need for greater information on the caseloads of district nurses. Attention has focussed on the number of patient contacts, but district nurses are unhappy with this measurement because it gives no allowance for individual patients' needs and can lead to decreased quality of care. Systems which are developing to record nursing need alongside quantity of contacts should receive more attention.

Article Abstract:
The UK Audit Commission has made recommendations to improve the efficiency of the district nursing system. Demands on the district nursing service are rising, but the lack of clarity on criteria for referral, and the district nurses' own uncertainty about their objectives, do not promote service efficiency. Other factors revealed by the review included caseload inequality, inadequate methods for managing caseloads, wide variations in assessment and management of patients, and inadequate oversight of clinical practice.

Testing the feasibility of timed district nurses' visits
Article Abstract:
District nurses have not normally operated a system of timed visits, but government efforts to improve patient services could lead to timed visits becoming more common. A recent pilot scheme has shown that it is possible for timed visits to be successful in a community context, as long as work levels are carefully planned. District nurses need to have very good organisational and management skills and adequate resources if a time visits system is to be really effective.
