More than skin deep
Article Abstract:
The general condition of a patient's skin should be taken into account in conjunction with other procedures for assessing their overall health. Skin condition can indicate personal hygiene levels, nutritional status, fluid intake and medical conditions such as pressure sores. Patients may suffer psychologically from a change in body image following chronic disease or traumatic injury and nurses should take this into account. The dermis inner section of the skin comprises a reticular layer and papillary layer, and the epidermis outer layer comprises the stratum basale, stratum spinosum, stratum granulosum, stratum licidum and stratum corneum layers.

Article Abstract:
Nursing home nurses need to develop a more holistic approach to dealing with wound care management of elderly patients. It is important to ensure high levels of patient hygiene, reducing the incidents of infection and time required for wound treatment. A full record of the patient's wound must also be kept to ensure continuity of treatment. Regular recording of wound dimensions and leakages will enable those providing care to assess level of pain relief required and bandage types.

A description is presented of the treatment and management of wounds affected by slough. Advice is given on the recognition of slough and on how to differentiate between slough and fibrinous exudate.
