Article Abstract:
Sexual dysfunction and infertility in patients with spinal cord injuries, treatment of low back pain and the care of patients with terminal illnesses are three areas of increasing research in physical and rehabilitative medicine. Male patients with spinal cord injuries (SCI) often have problems with infertility. There are a few methods that patients can use to stimulate ejaculation, though some arrangement must be made for insemination of a partner. Management of pregnancy in a female with SCI requires coordination between anesthetists and physicians familiar with SCI. Patients with low back pain often benefit from therapy that includes strengthening exercises and a thorough program to prepare them to return to work. Physicians who practice physical medicine (physiatrists) are also playing a larger role in helping patients with terminal illnesses maintain the best quality of life possible for as long as possible.

The role of physiatrists is expanding due to the development of non-hospital based rehabilitation services. Hospital-based services expanded in the 1980s when the value of rehabilitation was realized. However, the rise of managed care has led to the expansion of skilled nursing facilities and step-down units in hospitals. Because of this, many physiatrists have to work with primary care physicians, many of whom do not recognize the importance of rehabilitation. Survival rates after stroke continue to improve due to innovations in medical management. Back pain continues to challenge physiatrists, and there are many treatments. Many techniques, such as spinal manipulation, have not been proven effective. General physical fitness and flexibility can prevent many incidents of back pain.

Physiatry, the combined specialty of medicine and rehabilitation, has experienced a 124% growth in 10 years to 4,642 specialists. The number of rehabilitation services has expanded and the job of the physiatrist includes rehabilitation programs and care of patients with neuromuscular and musculoskeletal disorders. Patients getting physiatric care include those with lifelong disabilities such as cerebral palsy. Electromyography-triggered electrical muscle stimulation combined with muscle retraining can be effective in hemiparesis. Injection of botulinum toxin is effective in treating spasticity. A conservative approach is recommended in the treatment of lower back pain, including spine stabilization exercises and stretching exercises.
