The crisis in primary care and the role of medical schools: defining the issues
Article Abstract:
The number of medical students choosing residencies in primary care has increased, but more graduates of foreign medical schools are accepting these positions. Most American medical schools have courses or clerkships in primary care, but few American medical students choose primary care as a specialty. Many students incur huge debts during medical school, and must choose a well-paying specialty to support themselves. In addition, many students do not want to work the long hours typical of a primary care physician. They may also be turned off by the paperwork involved in a practice, as well as the need to care for patients with AIDS. Medical schools should recruit primary care physicians from the community to teach and serve as role models. The National Health Service Corps should be expanded, as should the number of Fifth Pathway programs. State and federal agencies could provide low-interest loans to build primary care clinics in underserved areas.

End-of-life care movement growing
Article Abstract:
Several organizations are developing continuing education courses to teach physicians how to provide adequate end-of-life care. Many of these initiatives sprang from the Study to Understand Prognosis and Preferences of Outcomes and Risks of Treatments (SUPPORT). This study found among other things that many of the 10,000 critically ill patients evaluated were in pain in their last days of life. Some of the organizations involved are the Education for Physicians on End-of-Life Care Project, the American Institute of Life-Threatening Diseases and the American Medical Association.
