Article Abstract:
UK nurses have an opportunity to develop their professional status through the Primary Care Act pilot site (PCAPS) scheme. The initiative allows nurses to develop and run primary health care schemes. Community nurses in the Lambeth Healthcare NHS Trust took up this opportunity and developed a PCAPS at the Edith Cavell Practice in inner city London. Patients who visit the nurse-led practice can opt to see a doctor or a nurse, but nurses envisage that they will undertake up to 60% of all patient contacts. Challenges, after three months of operation, include legal restrictions on prescribing.

An effective leader of a community nursing team requires a range of skills. Team leaders must be able to give the encouragement that is vital to the improvement of services for patients. They must also be part of the team as a whole, and should be accountable to other team members. They need the support of colleagues, but also benefit from access to support and advice from the professional development unit. Team leaders must be committed to a democratic and inclusive approach to decision-making, and must see themselves as a peer coordinator, rather than a hierarchical manager.

Intermediate care: smoothing the road to recovery
Article Abstract:
Health authorities in Liverpool, England, opted to change the use of a small community hospital which was being used primarily for social respite care. Managers considered a range of more appropriate uses for the old building and decided that intermediate care with clinical respite was the most appropriate and effective. The service, which frees acute beds in the area, is nurse-led. There are no doctors in the hospital, but nurses may consult a dedicated medical officer. Nurses make decisions about eligibility for admission after consulting with primary and secondary services.
