Piper Alpha
Article Abstract:
The explosions on the Piper Alpha oil and gas platform were caused by leaking condensate which was set alight by an unknown source. The Piper Alpha platform was located in the sea north of Scotland when in Jul 1988 it exploded killing 167 workers. An inquiry into the incident found that maintenance procedures concerning the pressure safety vales were not adequate. Safety routines were also discovered to be faulty as the neighbouring platforms continued to pump gas and oil to Alpha Piper despite evidence of an explosion and fire. Alpha Piper was demolished in Mar 1989.

Article Abstract:
Fines of up to 2 million pounds sterling resulted from an inquiry into the collapse of a ferry walkway in Kent, UK, which killed six people and injuried a further eight in September 1995. The fines were imposed by the Health and Safety Executive after hearing the port management company, Port Ramsgate, had claimed the walkway collapse was not its fault despite having commissioned a Swedish company to design and build it in 1994 at a cost of 1.45 million pounds sterling

Piper Alpha
Article Abstract:
The explosion on board Piper Alpha in 1988 and the subsequent inquiries and resulting recommendations, are reviewed. The events immediately after the explosion are described, as well as the permits to work system, operability of the fire-fighting system, emergency training and safety management. It concludes that the disaster was due to ineffective safety management, particularly with regard to the Permit to Work system.
