Annals' 70th anniversary: a look back and a look ahead

Annals looks back to the past and forward to the future on its 70th anniversary. From its beginnings in the 1930s, Annals has grown to be generalist medical journal that earns respect worldwide for its publication of high-quality clinical research and articles on relevant social and economic medical issues. Annals anticipates that its future growth and development will be in the arena of electronic communications. Annals is already available on CD-ROM and has a web site, and it hopes to develop new and better ways of taking advantage of the special capabilities of electronic systems.

Is managed care a "monstrous hybrid"?
Article Abstract:
Medicine needs to find some kind of balance between two moral syndromes: the guardian and the commercial. The introduction of commerce into medicine in the form of managed care has led to increased efficiency and lower costs but many physicians and patients are not happy with this situation. However, it seems clear that both moral syndromes are needed. A caste system is an example of an approach to maintain both syndromes but knowledgeable flexibility may be a better approach. This allows participants to use either syndrome as the need arises.

Databases in the next millennium
Article Abstract:
Harnessing the forces that drive many health care providers to apply available technology may be necessary to make database information useful into the future. Many current and past researchers have shown database analyses to be effective tools for evaluating health care policy. However, the application of this knowledge may be more complex. It may be wise for clinicians to keep database use in perspective, in terms of both cost and relative importance, and use caution when interpreting data derived from databases.
