Quality improvement principles in practice: the reduction of umbilical cord blood errors in the labor and delivery suite
Article Abstract:
The reduction of umbilical cord blood errors in the labor and delivery suite has been studied by an interdisciplinary group. The hospital information system was changed to achieve improvement and automating steps greatly reduced errors. Mislabeled umbilical cord blood specimens had been an important source of errors and presented a hard-to-solve problem. In the first full month after implementation no mislabeled specimens were reported. Involvement and commitment by key stakeholders and use of systems reengineering principles were important to changes achieved.

Article Abstract:
Three independent hospital departments cooperated together to streamline the documentation and review process for blood transfusions. One person from each group involved assembled to discuss how the current procedure could be improved. The result of their efforts was the creation of a new uniform document to be used by all nursing departments, Blood Bank staff, and quality assurance personnel. This reduced review and charting time and the number of documentation and procedural errors.
