Ramsgate walkway - 1.7m pounds sterling fines
Article Abstract:
Fines totalling 1.7 million pounds sterling have been imposed on Port of Ramsgate, Lloyd's Register of Shipping and two Swedish companies following their conviction for the collapse of a ferry walkway in Ramsgate, England, in Sep 1994. This led to six deaths. The court heard how the project to construct the walkway, which was only installed in May 1994, was very badly managed. The walkway collapsed because a steel pin came loose as a result of a basic design fault.

Article Abstract:
Employers will have to ensure that permanent warning labels are affixed to work equipment by the beginning of 1997 to comply with the Provision and Use of Work Equipment Regulations 1992 (PUWER). To determine whether warnings are appropriate, the risks of the equipment need to be assessed, and whether risks have been controlled by the design and construction of the equipment.
