Steroid induced osteoporosis: an opportunity for prevention?
Article Abstract:
Patients receiving corticosteroid treatment may not be receiving therapy to counteract the increased risk of osteoporosis. Pharmacology records from a three-month period from the Aberdeen Royal Infirmary in Scotland were reviewed. Corticosteroid therapy was common with 214 patients receiving them at a rate of 2.5 new prescriptions per day. Of these, 39.2% were prescribed for chest disease, 7.9% for cancer treatment, 7.5% for inflammatory bowel disease, 7.5% for rheumatoid arthritis or connective tissue disease, and 3.3% for kidney diseases. Only 5.6% received any type of therapy to prevent osteoporosis. High risk patients, such as postmenopausal women and patients receiving high-dose long-term corticosteroid therapy, were not more likely to be given treatment to prevent osteoporosis.

Prevention of glucocorticoid induced osteoporosis
Article Abstract:
Several guidelines have been suggested for determining the need for additional treatment with hormone replacement therapy or biphosphonates to prevent osteoporosis among patients treated with glucocorticoids. Baseline bone mineral density (BMD) measurements can be used to identify an immediate need for supplemental treatment and to track treatment progress. Patients most likely to need supplemental treatment include those with rheumatoid arthritis or asthma, who are at risk for falling, who take increased levels of glucocorticoids, or who have low BMDs. Healthy lifestyle choices should also be encouraged in treated patients.
