Taking the tube home: The development of a nutritional support team in a district general hospital in Dudley enabled patients with intestinal failure to opt for home parenteral nutrition
Article Abstract:
A multidisciplinary approach is required to manage intestinal failure. The nutrition team at a district general hospital in Dudley, incorporates a nutrition nurse specialist, dietitian and pharmacist, and the team aims to define problems, devise solutions and implement them via training, audit and direct involvement, so that patients are fed as appropriately, effectively and safely as possible. The enteral route is usually the first choice, and the use of endoscopically placed nasoenteric tubes is increasing. Most patients with intestinal failure require training for home parenteral nutrition before discharge from hospital.

Critically ill patients are very vulnerable to malnutrition, with only a moderate degree of weight loss potentially placing the patient in severe danger. It is important to take into account the patient's pre-admission nutritional status, and a full physical assessment must be made. Nutritional demands will vary according to the patient's physical state, with those suffering from major injuries or sepsis having greater nutritional demands than medical or surgical patients.
