Ten years after cryosurgical treatment of cervical intraepithelial neoplasia

Cryosurgery utilizes extreme cold for destroying or removing diseased tissue. A probe can be made very cold with nitrous oxide, which is used to destroy abnormal tissue. This technique has been used to treat cervical intraepithelial neoplasia, abnormal areas of tissue in the cervix that can lead to cancer. This condition is found in teenagers at a rate of 18.8 per 1,000; the abnormalities take an average of 10 years to develop into invasive cancer. The progress in 96 patients treated with cryotherapy for cervical intraepithelial neoplasia at least 10 years earlier was examined. The women were all treated between 1974 and 1977. They were reexamined periodically and were retreated if a lesion was found after the first year of treatment. Results showed that nine patients required retreatment after the first year, and six still had lesions after the second treatment, of whom three underwent a third treatment and three had hysterectomies. Ten-year follow-up was not available for seven patients. One patient continued to have lesions after three treatments and eventually developed cancer. Three patients had recurrent lesions at least three years after treatment. The overall cure rate was 92 percent. These results indicate that cryosurgery is safe and effective, as well as being gentle in treating cervical epithelial neoplasia. (Consumer Summary produced by Reliance Medical Information, Inc.)

Impact of therapy for cervical intraepithelial neoplasia on fertility
Article Abstract:
Little data exist on the impact of treatment for cervical intraepithelial neoplasia on fertility except for cone biopsy. Cervical intraepithelial neoplasia is an early cancerous condition of the cervix. Cone biopsies either by knife or laser do not appear to affect the ability to conceive but are associated with increased incidence of mid-pregnancy miscarriage, premature birth, and low-birth-weight infants. The effect depends on the amount of tissue removed. Loop electrical excision procedure, cryotherapy, laser vaporization, and diathermy cautery may not have adverse effects on either conception or pregnancy, but data are limited, especially regarding diathermy cautery.

Can the number of cigarettes smoked predict high-grade cervical intraepithelial neoplasia among women with mildly abnormal cervical smears?
Article Abstract:
Women who are heavy smokers may increase their risk of cervical cancer. Researchers evaluated the Pap smears of 173 women who had an abnormal smear. Compared to non-smokers, the risk of a precancerous condition called high-grade cervical intraepithelial neoplasia (CIN) was doubled in the women who smoked more than 10 cigarettes a day and the risk increased fourfold in women who smoked more than 20 cigarettes a day.
