Article Abstract:
Community psychiatric nurse Steve Clifford believes that his decision to become self-employed has allowed him to create an effective practice. He maintains that self-employment demands high levels of competence. Clifford, who works within the socially-deprived community of Hastings, England, believes that rapid intervention, within surroundings familiar to the client, produces the best results. He makes himself available at all hours of the day, sees clients immediately on referral if necessary, and spends time building up trust with clients.

Issues concerning the recruitment and retention of doctors and nurses to the armed forces are discussed. The role of the National Centre for Defence Medicine, Selly Oak Hospital, Birmingham, in training service personnel and stemming staff exodus is examined.
