The effect of managed care on ICU length of stay: implications for Medicare
Article Abstract:
Managed care does not seem to have reduced costs for elderly patients admitted to an intensive care unit (ICU). Researchers analyzed 104,270 adult admissions to an ICU in Massachusetts. Some of the patients were covered by fee-for-service plans, some were covered by managed care, some were Medicare beneficiaries and some were Medicare beneficiaries who had been enrolled in managed care. Managed care did not result in a significant reduction in length of stay, which is used as a measure of resource use. This has important implications for states that are shifting more Medicare patients into managed care.

Testing protocols in the intensive care unit
Article Abstract:
The challenges of the thorough assessment of increasingly sophisticated protocols for the care of critically ill patients are discussed. The focus is on two trials of alternate strategies for setting positive end-expiratory pressure (PEEP) in ventilated patients with acute lung injury or acute respiratory distress syndrome.

There may not be enough doctors who specialize in critical care after the year 2007. This will happen primarily because of the aging of the population, not necessarily because of a reduction in the supply of critical care doctors.
